Class Waiver:

I, the undersigned, am the parent or legal guardian of the child named above (hereinafter referred to as "Participant"). I hereby give my full consent and approval for my child to participate in the craft classes organized by Handicrafts Class LLC, to be held at Sauganash Park of the Chicago Park District.

I understand that there are inherent risks associated with any craft activities, including but not limited to the use of scissors, glue, paint, and other crafting tools and materials. I acknowledge that while Handicrafts Class LLC and its instructors will take all necessary precautions to ensure the safety of the Participant, accidents and injuries can occur.

In consideration of allowing my child to participate in the craft classes, I hereby agree to the following:

Assumption of Risk:

I assume all risks and hazards incidental to the conduct of the craft classes and activities. I hereby acknowledge that my child’s participation in these activities is voluntary.

Waiver of Liability:

I waive, release, absolve, indemnify, and agree to hold harmless Handicrafts Class LLC, its owners, agents, employees, instructors, volunteers, and the Chicago Park District from any and all claims, damages, losses, or injuries arising out of or related to my child’s participation in the craft classes, including but not limited to any injury or accident that may occur.

Medical Treatment:

In the event of an emergency, I authorize Handicrafts Class LLC and its representatives to seek medical treatment for my child if necessary. I understand that I will be responsible for any medical expenses incurred.

Photographic Release:

I grant Handicrafts Class LLC permission to take photographs and videos of my child during the craft classes for promotional and educational purposes. I understand that these images may be used on the company’s website, social media, and other marketing materials.

Behavioral Expectations:

I acknowledge that my child is expected to follow all instructions and behave appropriately during the classes. Disruptive or unsafe behavior may result in the Participant being removed from the class without a refund.
